ABSTRACT

OBJECTIVE:To investigate the relationship between HLA-B*5801 gene polymorphism and allopurinol-induced ADR in the Han population of Hainan Province. METHODS:The in-situ hybridization fluorescence staining analysis technique was used to detect HLA-B*5801 allele of 149 inpatients receiving allopurinol in Hainan Provincial People's Hospital during Sept. 2015-Sept. 2017.They were divided into tolerance group and ADR group according to ADR.Woolf's formula was used to calculate OR. The correlation of HLA-B*5801 allele with the occurrence of allopurinol-induced ADR was analyzed. RESULTS:Of 149 patients,there were 133 cases in tolerance group,among which 17.29%(23/133)carried HLA-B*5801 allele.There were 16 cases in ADR group,among which 93.75%(15/16)carried HLA-B*5801 allele. Among 16 ADR patients,13 patients suffered from lesion of skin and its appendents;1 patient suffered from systemic damage;1 patient suffered from gastrointestinal systemic damage;1 patient suffered from central and peripheral nervous system damage. The risk of ADR in patients with HLA-B*5801 allele was significantly higher than patients without HLA-B*5801 allele(OR:71.74,95%CI:9.02-570.55,P<0.000). The lesion of skin and its appendents was strongly associated with HLA-B*5801 allele(OR:57.39,95%CI:7.11-463.50,P<0.000). CONCLUSIONS:HLA-B*5801 allele is strongly associated with allopurinol-induced ADR. It is suggested that HLA-B*5801 allele of Han patients should be detected before taking allopurinol,which helps to reduce the incidence of allopurinol-induced ADR.

ABSTRACT

Objective To investigate the clinical efficacy of vertebral pedicle screw fixation for lumbar fractures without nerve injury. Methods A total of 100 patients with vertebral fractures from February 2012 to December 2012 in our hospital were researched.According to the order of the patient admitted to the hospital,50 patients from February 2012 to June 2012 were divided into observation group,50 pa-tients from 2012 July to December 2012 were divided into control group.The observation group were treated with vertebral pedicle screw fixa-tion,the control group were treated with a four nail fixation.The therapy and complications between two groups were compared after treatment, and the satisfaction of two groups were investigated.Results There was no significant difference in vertebral anterior height compression rate and Cobb angle between two groups before treatment(t =0.356 9,t =0.633 6,P =0.721 9,P =0.527 8),but after a week of treatment,ver-tebral anterior height compression rate and Cobb angle in observation group was better than those of the control group,the difference was sta-tistically significant(t =2.705 7,t =2.840 2,P =0.008 0,P =0.005 5).After one-year follow-up,there was no significant difference in effi-cacy between two groups(t =0.914 7,t =0.464 6,P =0.362 6,P =0.643 2).The postoperative complication rate of observation group was 4%,compared with 16% of control group,the difference was statistically significant (χ2 =4.000 0,P =0.045 5).The satisfaction rate of patient in observation group was 96%,compared with 84%of the control group,the difference was statistically significant (u =2.477 9,P =0.013 2). The fracturesof patients in two groups after 12 months were cured,without aggravating circumstances in nerve damage .Conclusion The ver-tebral pedicle screw fixation can effectively correct angular deformity,restore vertebral body height,strengthen the stability of the spine for treatment of patients with lumbar vertebrae without nerve injury.

ABSTRACT

Objective To analyze the results of treatment of complicated tibial plateau fractures with open reduction and internal fixation with bilateral plates .Methods 29 elder patients with C type tibial platform fractures underwent internal fixation with bilat-eral plates and bone grafting .Fixation with bilateral plates was performed during operation with dynamic compression plate or 1/3 Tubular steel plate placed medially and the dissection plate or LISS system ones inserted laterally after the underlying separation . Meanwhile ,sufficient bone grafting during operation could effectively support reduction of articular surface .Results All patients were followed up of an average of 13 .8 ± 2 .43 months .According to Rasmussen score standard ,the excellent and good rate was 82 .7% .There was significant difference of TPA and PA between postoperation and postoperation one year later .Conclusion It is a simple and effective method to treat complicated elder tibial platform fractures with bilateral plates .It will improve the effect and re-duce complications significantly to perform sufficient bone grafting intraoperatively ,and make the drainage unobstructed and do the early functional exercises postoperatively .

ABSTRACT

<p><b>OBJECTIVE</b>To observe the role of green fluorescent protein (GFP) in tracing rhesus bone marrow stromal cells (rBMSCs) during tissue-engineered bone formation in vivo.</p><p><b>METHODS</b>Ad5.CMV-GFP was amplified by infecting QBI-293A cells, and the bone marrow was harvested from the ilium of adult male rhesus to obtain rBMSCs, which were cultured and passaged in vitro. GFP was transfected into the third-passage rBMSCs via adenovirus vector and the labeled cells were inoculated into absorbable HA scaffold and cultured for 3 days, with untransfected rBMSCs as control, before the cell-matrix compounds were implanted into the latissimus dorsi muscles of rhesus. Samples were harvested at 6 week and embedded in paraform, and ground sections of the bone tissue were prepared to observe green fluorescence under laser scanning confocal microscope. Propidium iodide staining of the sections was also performed for observation.</p><p><b>RESULTS</b>The rBMSCs grew well after GFP transfection, and green fluorescence could be seen 24 h after the transfection and became stronger till 48 h, with a positive transfection rate beyond 80%. Six weeks after cell implantation, the rBMSCs labeled by GFP-emitted green fluorescence were detected in the bone tissue under laser scanning confocal microscope.</p><p><b>CONCLUSION</b>GFP can effectively trace BMSCs during bone tissue engineering, and the transplanted BMSCs constitute the main source of bone-forming cells in bone tissue engineering.</p>

ABSTRACT

Objective To investigate an effective method for treatment of scaphoid nonunion.Methods Based on the applied anatomy study of 12 fresh upper limb specimens of human adult cadavers,a new transfer of vascular pedicled radius with the 1st,2nd intercompartmeiilal supraretinacular arteries has been designed to treat nonunion of scaphoid fractures in 23 cases since 1997.Results All cases were followed up from 6 months to 3 years and their fractured seaphoids got united 3 to 7 months after the operation.The bony union rate was 100%.Rate of excellent and good restoration of normal wrist function was 95.7%.Conclusion This method for treatment of nonunion of scaphoid fractures is re commendable,for it is simple to manage,less invasive,and effective.
